| Race Information Pre-race Every year, I go on an adventure for my birthday. This year, I decided to do a h lf Ironman. I began training two months before race day. My baseline fitness was already high, having played five varsity sports in high school and competing on the water polo team at Carnegie Mellon. I knew I could finish the race, but finishing was not my goal. Instead, I chose the following times: However, two days into training, I fell in downtown Pittsburgh and found myself on crutches with a high ankle sprain. I hobbled between classes, knowing that quitting was not an option. I couldn't run, but I could certainly swim and bike. And so I began long rides through the woods and swim sets after class. A week before the race, I flew back to California. Training along the sunny coast was refreshing after many cold days traversing Pittsburgh's cobbled hills. Gear Shoes The Altra Escalante 3: the bluebirds that would fly my feet to the finish line. Bike I rolled up to transition with a bike I purchased three days ago. My first clip-in bike. Nothing fancy or schmancy. I had ten miles on the bike when I pushed my treaded tires into transition. I installed a water cage on the morning of the race. With limited time and tools, the cage was hanging on by one screw. Wetsuit I used a Roka Maverick wetsuit. Before the race, I had five minutes of swimming in the suit. After dealing with an initial sense of claustrophobia, I came to appreciate the increased buoyancy and warmth provided by the suit. Race Day Nervous energy was zipping through the crowd. As I got closer to the start, I could hear the automated beep that released every three competitors into the chilled water of Morro Bay. I was calm. Hi all,
Over the past two years, I've been tracking my extended high frequency thresholds using calibrated audio gear at home. I've noticed two things:
1) Despite being in my twenties, my maximum audible frequency is about 14kHz in both ears. I understand EHFs are extremely variable by individual, but for my age this still has me concerned.
2) Throughout my 2-year tracking, I've noticed unpredictable fluctuations in my top-end EHF thresholds, despite testing regularly and obtaining consistent baselines. I will often check after being in a setting of loud noise, and the thresholds will be similar to baseline, yet one random evening I'll test and 14kHz in one ear will be down 10db or more. I understand this is within test-retest, but this loss at the top end of my hearing is consistent if I test multiple times a day or over a given week, and immediate lower EHFs remain similar to baseline. I've not been able to find if this is normal in other individuals, and I am left curious.
This week, I had another 10db drop in my right ear at 14kHz with no explanation. Just to experiment, my neurotologist and I decided to try 30mg of Prednisone, but the hearing did not return. I imagine I am close to losing this frequency.
I have two questions:
1) What do you make of these EHF fluctuations, given they are always only at the highest frequency I can hear? My leading guess would be noise-induced hearing loss, but the spontaneous drops and occasional jumps back up have me second guessing.
2) Given I am young, I want to be vigilant of how this hearing loss may progress, but the spontaneous fluctuations have me and my neurotologist puzzled. I want to be realistic but need to understand better: How steadily would the hearing decline? As the hair cells are arranged tonotopically, would you expect the decline to slow as it makes its way to 13kHz, 12kHz, etc? I would love an in-depth explanation on the behavior and rate in which hearing loss (primarily from noise exposure or presbycusis) works its way through the cochlea.

2023.06.07 09:12 Turbulent_Back3287 How is it that taking a drug as prescribed makes you less likely to develop a tolerance, and some people can stop taking theirs without experiencing withdrawal?
So to start off I'm going to explain my understanding of drug tolerance. When you take a substance that has an effect on how your body functions, e.g. by increasing dopamine in your brain, your body decreases whatever mechanism already created that function to compensate for the drug. So if your baseline is 100% and you begin taking adderall that puts you at 150%, your body will slowly decrease it's functioning by 50% so you can return to baseline. If you then stopped taking the adderall, you will be at 50% until your body adapts to the lack. The period of time before that happens is withdrawal.
I've been on adderall for ADHD for a year and went through titration, during which I did a lot of research because I like to understand things. I think I'm missing some information about how drug tolerance works. Basically every resource with information on adderall has a prologue to remind you that abusing it is very naughty and lists all the bad things that might happen if you do. One of those is a higher potential for tolerance/dependency. That doesn't make sense to me because I would imagine abusing adderall means you have a less consistent supply of it, unless drug dealers are somehow more reliable that US pharmacies. If you're not taking it consistently, wouldn't your body would adapt to it less quickly since you're still having days that you're not on it? Maybe it's referring to the fact that nobody is monitoring how much you're taking, so as you develop tolerance you'll start taking more
Another thing I don't understand is how it's possible to suddenly stop taking it without experiencing withdrawal. There have been a few instances where I haven't been able to refill my prescription for anywhere from a few days to a week, during all of which I had to work, and I was afraid I was going to be going through withdrawal on the clock. I mentioned this to my stepdad and he said him and his son both took adderall at some point, and when they stopped nothing happened. And to my surprise I didn't experience anything either, except being non medicated and not being used to that anymore. The effect adderall has on me is less pronounced and I've been taking it long enough that I must have some tolerance, so why didn't I feel ill? Is it that your body is able to adjust instantly if the amount of substance you have tolerance for is small enough?

My best bud aka 16 year old cat Chunker was acting a bit off and drooling a lot over the past few weeks. I thought he needed dental work, and made an appt to have a teefies exam at the vet. However, the day before our appointment his posture got really weird, like he was in pain, so I rushed him at 2AM to the emergency vet.
One extremely expensive vet bill later, I found out Chunker has stage 3 kidney disease with a 3.1 creatine level. I was shocked, because he had his senior bloodwork done 10 months prior and he was totally fine. They wanted to hospitalize him overnight with IV fluid, but I elected to take him home and do multiple rounds of subcutaneous injections. To be honest, I couldn't afford the vet bill to hospitalize him -- it was the same cost as 3 months rent!
I was feeling really guilty, but after a few rounds of subcutaneous fluid he perked up! Today he climbed to the top of his 5 foot tall cat tree, and has begged for people food which is business as usual for Chunker. A few questions I have for those more experienced:
-Chunker is turning up his nose at the prescription kidney diet food. Weirdly, he loved the urinary tract food he used to be on. Can I do something to make it taste better? Crush up bonito flakes?
-Chunker is still drooling A LOT. Like his entire chest is constantly wet. Is that normal? I'm having his teeth looked at next week -- I don't want him to have an infected tooth on top of this, or be in pain.
-The first day Chunker came home I tried the Gabapentin as prescribed since I thought he might be in pain. It made him very wobbly and punch drunk. He was so upset he kept trying to run out the front door, which he never does. Is this reaction normal?
Any and all advice for Chunker appreciated. He is the bestest boy and I want to make him comfortable, but also, try to raise his baseline health level.

After doing this for a couple of weeks my brain felt like I've had 23 coffees all the time and my sleep is non existent. I've gone cold turkey for two days and feeling less like my head is attached to a rocket, but wondering what the washout period might be before my serotonin/brain chemistry return to a baseline as I'd like to get back on NAC only.

I called PVSEC (the emergency vet) and they said they were at capacity for the night and would only accept them if they were critical. Of course I didn't want to chance it and brought them in anyways and here we are... I always get so depressed waiting here and seeing all the sad pets and families that come through. I can tell it's going to be a long night ๐
Narwhal, Minke, and mama Bryde seemed to have better energy and were eating, so we'll have them seen in the morning. They all received their first rounds of FVRCP vaccinations on intake so hopefully they have some protection against some of the nastier viruses...
This kitten season has been really rough and this is so disheartening... We love these guys so much!

Hey. I've got several chronic fatigue causing issues going on, and never felt like ME/CFS fit into the puzzle, especially since I feel good after exercise, including everyday physical exertion.
That said. I've recently been reflecting on the fact that the way my cognitive fatigue begins to ramp up from any cognitive effort, and the way pushing through it always ends really poorly, complete with doing worse on subsequent days and the fact that almost all my "doing genuinely better" periods were right after a week+ of not pushing myself whatsoever... I'm starting to think this is neither a mental health thing nor baseline fatigue, but rather fits the description of PEM really well.
It's not strictly only after cognitive exertion, though, I get this after an orgasm, and heavy physical exertion on an empty stomach(I fast until evening due to GI issues) can get me there too.
Worst cases I'll spend several hours unable to even think, extremely broken, from which I recover very slowly and have reduced tolerance for exertion for days at least.

Police were called and the person escaped through the Delcourt(delmont?) apartments.
He has short hair, beard, baggy (red?) and black sweater, some sort of orange shoes or cloths close to his feet.
This happened today around 9:00 PM

I get a lot of good information and tips from you BC friends. Thankfully, one of you mentioned the benefit of getting a pre-mastectomy baseline physical therapy session, and someone else mentioned developing lymphedema after just one node removal. This prompted me to spend a few hours researching local physical therapy options and I was extremely fortunate to find a PT experienced in BC, who re-arranged her schedule to get me in prior to my upcoming mastectomy June 15.
By the way, neither my plastics surgeon, onc surgeon or PCP had even brought up the subject of PT, much less pre-surgery.
I visited her today, while I am pain-free and clear thinking, to establish care and get some education. She did detailed baseline measurements of my fingers and various points on my arms, and strength testing, and she recommended a good compression bra for post-surgery (I thought what I had was adequate, but not so). We discussed osteoporosis risk reduction while taking AI, and she was so optimistic and gave me hope that she will help get me through this.
She picked up that I was dreading AI and encouraged me to take advantage of free cancer counseling in my area that I had not been aware of. I think I am going to go for it... I feel the need to talk about what I am going through more than my friends really want to hear.
I am thankful for everything I am learning on this sub-reddit, and if anyone has the opportunity to to have a pre-surgery PT session, I would recommend it.
