Hello everyone,
Sorry for the long post, but someone please help me with suggestions or information, I am desperate for pain relief! I am a 28 year old male, considered 100% disabled by DOD after 10 years of being in the military. I have Spondylolisthesis, spondylosis, spinal stenosis, ankylosing spondylitis, herniations at L4-L5, L5-S1, T8-T9, C4-C5, arthritis in multiple facet joints, both knees very arthritic with severe crepitus, tendinitis, a couple of cruciate ligament injuries and meniscus removed from the left, along with right hip arthritis, chronic migraines, labral tear and impingement in right shoulder- among many others. I hit the jackpot with chronic pain, which I’ve had for 8 years now. I started on chronic opioid therapy 7 years ago, which has literally kept me alive. Before my last deployment I was at a 65mg MME (ER Morphine and hydrocodone). When I came home, I ended up switching to Buprenorphine (titrated to 24mg) and Tramadol for breakthrough (I understand Buprenorphine blocks the opioid receptors for the most part, but Tramadol’s mechanism of action with Serotonin was helping my pain noticeably more, plus I absolutely was still getting some of the opioid receptor relief, just not all) because my pain was just getting much worse. Now, almost two years later, I have switched back to full-agonist opioids, currently ER Morphine and Norco again, 30mg and 15mg/24hrs respectively. I withdrew pretty hard from the Buprenorphine for about 11 days, I knew what to expect and got through it (I hate doing months of tapering, I would much rather do a 3 day taper and precipitate withdrawals to get it over with ASAP, pain management doc was okay with this) but now that the Buprenorphine is all out of my system, this dose is not only not controlling my pain very well at all, but I am in a constant state of mild withdrawals. I believe it is because of Buprenorphine’s absurdly high MME (750mg MME for the dose i was at) pushing my tolerance up quite a bit. I know with partial-agonists it is not a direct/exact 750mg MME, so obviously my body is still not in need of even 1/4 of that MME to meet my tolerance levels.
I am wondering what a reasonable/expected dose titration or medication switch could be reasonably expected in this situation, where my daily COWS score is 24-34, and pain level is 7-7.5 on average.
Would it be absurd to ask to increase from 15mg MSContin BID to 15mg TID (Q8H) if I am having end of dose failure at ~9 hours, and potentially switch the Norco 5mg TID to Oxycodone 5mg TID for the slight MME increase and higher bioavailability? That would be an MME increase of 22.5mg, from 45mg to 67.5mg. Closer to where I was at before Buprenorphine, to not only account for getting me out of withdrawals, but also treating my pain effectively. Or is something like 10mg ER Morphine TID/Q8H and 2mg Dilaudid IR TID more reasonable to ask about at 54mg MME? I have never tried Hydromorphone so I do not know how effective it is known to be vs. Oxycodone.
I would love to try Levorphanol being the much safer sister to methadone with it’s 8-12 hr half life instead of a stupidly unpredictably long half life and no QT prolongation, and even stronger NMDA receptor activity, but it seems it is such a rarely used medication that most doctors are quite unfamiliar with it and it’s extremely difficult to find a pharmacy that carries it or can order it, but my insurance will cover it.
Any advice, suggestions, or experience would be extremely appreciated! Thank you!

"There is mild joint space narrowing within the medial tibiofemoral compartments bilaterally. The joint spaces of the lateral tibiofemoral and patellofemoral compartments are well maintained. No erosions to suggest an underlying inflammatory or crystalline arthropathy. No fracture. No aggressive osseous lesion. Small amount of fluid within the suprapatellar bursae bilaterally."
Suspected hypermobile Ehler's danlos syndrome undiagnosed so far.
History of being very hypermobile in knees with constant pain bilaterally, and frequent patella tracking issues and subluxations. Clicking/grinding when squatting to standing up, visibile swelling in knee externally. Take celebrex 200mg 2x/day additional to glucosamine an chondroitin supplements daily as I have mild osteoarthritis bilateral in hips I found out in February.

I have osteoarthritis that my rheumatologist characterized as polyarthritis since it is everywhere in my body. I had a knee replacement last year, and I'll need to have the other knee done eventually. The bones in my feet hurt, my hip joints hate me, and my spine is unhappy. I deal with all of those things pretty well, but it's the situation in my hands that affects me most throughout the day--especially the basal joint arthritis.
I take 400mg of Plaquenil daily. I take Aleve most days, and arthritis-strength Tylenol often. I use Voltaren gel on my hands on the worst days, but I swear it's a placebo as it doesn't help me at all. My wrists are mostly fine, but my hands are not, not at all. Is there anything that really works to bring comfort? I have gardening to do, cooking, and hand-centric hobbies. I'm sad with the way things are going.

I sent a photo to my mother asking if she could demonstrate the shoulder hypermobility that I can do easily - and she can't, but both my little siblings (22M, 18F) can. Totally fine. I only asked to see if my hEDS was passed down as opposed to a genetic hiccup, and what this means is that it's probably my dad who also had it. One friend of mine is reasonably horrified at the photos, another one just texts me "Uh oh" with a photo of his flying bird hands and an inch and a half of pinched skin on the back.
I go into a discord server with some friends who share an unrelated psychiatric disorder to joke about "scaring friends and family in the Control Group with my creepy weird joints and accidentally diagnosing the rest" and somebody immediately says "But I have NoRmAl MoBiLiTy and I can do that 🤔"
Bestie. I hate to be the one to break it to you but prayer hands behind your back is not indicative of normal mobility, hEDS or otherwise.

Hello. My name is Steven(Male). I live in Long Island, New York - I am 29 years old and I need a kidney. I have lupus nephritis which has caused stage 5 kidney failure and a whole host of other medical issues, such as joint pain, fatigue, depression, anxiety, edema. I am currently on Peritoneal Dialysis(PD) 7 nights week while still working full time. I am currently in the process of being placed on a transplant list with New York Presbyterian - Weil Cornell Medicine. My insurance is Emblem Health HIP HMO.
Due to being O-Positive, I was informed by the doctors that my average wait time for a Kidney is 5-8 years, if not longer, if I’m not mistaken. That’s way a living donor is preferred and so necessary. For those who may not know and for those who are living with kidney failure, this has been a long, difficult, unpleasant, and exhausting battle. This whole ordeal in all has affected my mental health, my self worth and has impacted all aspects of my life. I feel as if I don’t have a future.
But receiving a kidney is more than just about me - It’s about my mother and my fiancé and the rest of my family. My mother had a stroke in 2014, and I am her sole caregiver since my brother and father have passed away (2017 and 2022 respectively). She helps as much as she can when I get home but, I don’t want to her to exhaust herself for me. She needs me, just as much as I need her. My fiancé and I have recently gotten engaged and I look forward to having more experiences with her and getting old with her. While she’s had battles of her own, she’s doing all that she can to support me mentally, but I don’t want to make her feel overwhelmed. She needs my support, just as much as I need hers. Between working full time and being on dialysis, I don’t have much time with them anymore, especially my fiancé (since we don’t live together). I miss their company and I need them to be ok. I want more opportunities to show them how much I appreciate them and love them.
I want to be here for the people that I love, I love to cook and I want to be able to cook every night and not be confined to a machine in my room. I want to be able to see my cousins grow up and graduate. I want to be able to go back and get my masters degree and be able to focus. I want to exercise again, daily, without feeling exhausted. I want to be able to feel free again. I want to be able to travel, without lugging around a machine and being confined to dialysis every evening. I’d love to take normal showers, jump in pools and feel the ocean again.
Though many times, things feel bleak, I’m willing to keep fighting And hope is not lost - Not for any of us facing this same challenge. I hope this reaches someone. I feel like this is going to be long so please bear with me. 33F Caucasian. 5’9 215lbs. I’ve never smoked and drink very rarely. (Mostly special events) l didn't see a PCP regularly up until this past year. Since seeing my new PCP I've been diagnosed with a bulging disc, anemia, vit D deficiency anxiety and depression. The anxiety and depression is mostly related to a recent trauma. Medications: Zoloft 50 mg daily, vit d 2000mcg daily, iron daily and skelaxin 800 mg TID PRN. I work third shift. I feel like I frequently brush off symptoms assuming that it's related to being a night shifter. I'm always exhausted. I could stay up for over 24 hours or sleep for 12 and I feel equally exhausted. I frequently find myself wasting my days off on the couch feeling too exhausted to move. I have a constant list of tasks flowing through my brain and can’t seem to get up to do it. This complaint is what lead to us finding the anemia and vit D deficiency. For 2 or 3 months l felt amazing. And now it's like I'm back in a slump again, same with the Zoloft. I felt so good for about 2 months and now I'm back to feeling the same. Other symptoms, swelling. My very visit with my PCP my ankles and feet were so swollen my shoes left indentions. She said we would monitor it. But now I'm noticing swelling in my hands when I wake up and randomly throughout the day. But my normally loose wedding ring was squeezing so tight my finger was tingling. I haven’t been able to wear it since around Mothers day. I have super frequent headaches. If you're around me I'm probably complaining about my back and my head. I'm constantly sick. Every time you turn around I have the flu or strep or some respiratory virus. I'm experiencing shortness of breath with exertion. But my 02 sats do not drop (I have a pulse ox left from when we had covid). I’ve been having a ton of nausea. Mostly after I eat. Last night I almost passed out. I was at the stove cooking and suddenly got very hot. I felt like I was burning all over, I was very lightheaded and my vision started getting fuzzy/fading out. I sat down and immediately felt like I was going to throw up, I was also shaking quite a bit. I laid back and the feeling passed and I was able to get up and continue on as normal. I just feel like this is such a wide variety of symptoms I’m not sure if they could all be related. One of my nurse friends is convinced I have something autoimmune going on. In my 20'S I was checked for rheumatoid arthritis because of frequent joint pain. I had also been diagnosed with bursitis of my hips and something with my knees that I don't remember the name of but it was swelling that was pushing my knee caps outward. My tests at that time came back negative.
I have reached out to my PCP for an appointment and am waiting to hear back for scheduling. I’m just looking for advice on if anyone thinks these could be related or if I’m overthinking. I don’t want to go into my appointment and sound like a crazy person or one of these people that tiktok has convinced they have every disorder under the Sun. I appreciate if you made it this far!

Any recommendations on other workout videos or programs that focus on strengthening to address SI pain? Would love something more fast paced and challenging, or with bands or other props.
I'm also on the lookout for Peloton barre workouts that focus on clamshells/hip bridges/glut medius exercises (without any pesky prone back extensions... Hannah Corbin loves those).

Recently I’ve had a really bad cough resulting in a sore throat. When I cough or sneeze it really hurts right above my hip bone. It even hurts when I laugh or clench my stomach. I did also have really bad pains in my stomach right under my ribs last week. They were so bad that I couldn’t move without pain. The area is also a little firm, and I don’t know if I should be worried.
update: the pain is still in the same spot, but is also moving to both of my hip joints.

The pain started near my knee, but it’s moved up my leg a bit into the middle and is kinda shooting up my leg a bit. I’ve had similar pains in my shin on the same leg, but none that hurt this much. It’s very painful and i don’t know how to get rid of it. It doesn’t hurt any more or less when i press on the area, so could it be the bone that hurts? I don’t know what to do because the pain usually goes away in 20-30 minutes but it’s been much longer than that.
update: the pain is still in the same spot, but is also moving to both of my hip joints.

Underlying condition (finally diagnosed?)
- Extremely weak left leg (glutes, quads, hips just don't get stronger)
- Identified knee ‘laxity’ (instability) causing tendonitis and all sorts of issues as explained in the MRI findings (see attached)
My history
-32 year old male, who relatively played no sports while growing up. Fell off a cycle and had stitches right on top of my knee when i was 13
-Got into fitness 5 years ago, lost 20+ kilos and got into long distance running and HIIT workouts -Mistakes made on the way, never focused on strength programs, unstructured plans, ran without any planning and in 2019 one of my PT’s noticed ‘my left leg is weaker than my right’ one
-Never paid attention until my first experience with ITB syndrome in 2020
-Focused on strength and started running again but no consistency due to constant niggles and set backs
-Sep 2021 second incident with ITB syndrome, physio finally said go to a sports doctor. Reputable sports doctor suggested steroids which i rejected and said ‘it is normal to have ITB syndrome’
-1.5 year in, did multiple strength programs, had periods of relief but I have NOT fixed my leg issues
Where i am today
-i can barely do a full lunge, constant pain (3-4/10) even after walking 10k steps

-Any bending flares up the knee
-Left hip is tight i.e. doesn’t move freely while doing IR’s (windshield vipers)
-Even getting off a chair and having a relatively easy knee bend is hard before i take a couple of steps
- I can't even think of any repetitive exercises such as running, elliptical etc etc
Apparently, I pronate slightly inwards and apparently i have flat feet.
My issue:
- Spent 1000's of dollars trying to fix my agony with no Success. Can this program really prove to the circuit breaker?

2023.05.30 06:19 Lee_street Question about possible subluxations?? (Attached Video)

I’m suspecting hEDS and already meet the criteria for criterion 2, Feature A and C on the Ehlers-danlos society’s diagnostic criteria. I’m stumped about the question regarding dislocations/instability. I’m fairly young, just turned 18, and know hEDS tends to be progressive so didn’t think this applied to me currently.
But my hip does something odd where it snaps back seemingly out of the joint, often audibly and very obviously visibly, but it’s painless and usually actually relieves pain. (It can happen when I step or be manual) I don’t know if it is a subluxation because I often hear about how subluxations are painful. But my brother is convinced that I also manually induce shoulder subluxations, I’m more certain I’m just popping them to reduce pain in those joints.
But that all leads to my question, can subluxations be painless?? And does the attached video possibly seem like one ,or, at the very least instability.
